Hello, I'm trying to fill out the rebate form in this promotion: http://www.asrock.com/events/usretailer/newegg-AM4-May/
The instructions state: "The name and address on the rebate form must match the purchase sales/invoice receipt or the rebate will be denied."
 
This is where I get stuck. My newegg invoice has a "Shipping Address" and a "Billing Address." Both are completely the same in every way except my shipping address abbreviates my first name.
 
Shipping Address = F. Last
Billing Address = First Last
 
Which one should I choose when filling out the rebate form so I do not get denied?
