RX5600XT PGD3 6GO Black screens

      Hello, 
 I bought the RX5600XT PGD3 6GO graphics card on 11/27/2020 and a few months later started to get black screens that could only be fixed by resetting my PC. After searching around I found that removing the AMD adrenaline software and  only using the drivers fixed the problem for a lot of people, and it did for me...until recently; I started to get black screens after cold boots, then during normal operation of my PC (browsing the internet, playing certain games etc)

 I have attempted to fill out the support form here:

 https://event.asrock.com/tsd.asp

but am given an error every time I enter the cards model number into the required field and also haven't had any response from your e-mail address under the RMT section of your website.
