I bought a USB 2.0 bracket which has 4 ports, 2 ports per a motherboard header. Here's the link:

https://www.amazon.com/StarTech-com-Port-Female-Plate-Adapter/dp/B00015Y0FK/ref=sr_1_1?keywords=StarTech.com+4+Port+USB+A+Female+Slot+Plate+Adapter+-+USB+panel+-+4+pin+USB+Type+A+%28F%29+%28USBPLATE4%29&qid=1572243284&sr=8-1

One of the twp USB motherboard headers doesn't work - (USB-3-4) - it's on the left. A flash drive connected to it blinks while booting but no device connected to it works.

I tried swapping the two header connectors and the same cable works on the other header. So it's definitely not the cable. Is that a defective motherboard?

From my point of view one thing you can do is to check in the bios, if there is something disabled for the XHCI controller.
Somewhere like \Advanced\AMD CBS\FCH Common Options\USB config

The other thing is to inspect the mainboard connector and the backside of the PCB, visual and with some measurement (if you are able to do so or you know somebody with the related experience)...

But it looks like this header is defective.

Those options are set as AUTO which is default option. Should I try ENABLE?

Should I try Clear CMOS?

I think load bios defaults should be ok too, but save your personal settings before (if you have some). Clear CMOS at least must be used, if you cannot reach the bios any more (for example, after a wrong OC try).

Yes, i would try to set it to ENABLE.

And you should contact Asrock Support, if ENABLE does not help.

I had the same problem as you with a different ASRock mobo: X570 Extreme4.
I think this is not a mobo defect but a design defect or some configuration issue that we can not change.

I was never able to make it work.
