USB 2.0 ports stopped working-ASRock P67 Pro3 SE

All of my USB 2.0 ports have stopped working, on the motherboard and on the case. The USB 3.0 ports work. Power is getting to the USB 2.0 ports and during repairs, I was able to get them to function but once Windows 10 got loaded, they stop working.

I've updated my drivers for my motherboard, chipset and BIOS. I've installed older verions and new versions of the chipset and drivers. I've run windows repair and windows troubleshooter. I've uninstalled everything USB and then restarted to try and get the real ones to pick up. I've uninstalled the windows update. I've suspended USB hibernation. I've checked the registry {36fc9e60-c465-11cf-8056-444553540000} for any filter entries.

The motherboard is ASRock P67 Pro3 SE.

I've attached a screen shot of what I see from device manager. Also under my Devices I see a device that is an Unknown USB Device, while the only USB Devices I have plugged in the USB 3.0 ports and are recognized.
