I've got nothing new for you, except making sure your HDMI cables are of good quality, and at least HDMI 1.4 compliant.

Can you set the vertical frequency to 135KHz? That apparently happens when you set your monitor itself,  HDMI ULTRA HD Deep Color set to ON. That's no doubt in your monitor's own settings. Or did you already do that?

I don't know how your board's HDMI2 port supports 60Hz at 4K resolution, since that is beyond the specifications of your processor, and any Skylake processor. Check the Graphics Specifications:

Also, if I change the resolution to 1920 * 1080, then I can run 60hz just fine. No go at any resolution over that and 60hz.

for completeness, I am using an i3-6100 on the Fatal1ty Z170 Gaming-ITX/ac.
latest intel graphoics drivers installed, two lg 27UD68-w monitors wirth their drivers installed as well.
The trick to enable the refresh rates over 30hz on HDMI is to set "deep color" to "on". However, when I do, I am met with a screen like this. (excuse the size)
Does this look familiar to anyone?

There are two Z170 ITX model boards, assuming you have the Fatal1ty Z170 Gaming-ITX/ac.

No mention of the CPU you are using, I hope its graphics core is up to driving two 4K displays.

Also note the resolution on HDMI2 is not the same as DisplayPort. HDMI2 is 4096x2160, DisplayPort is 4096x2304.

You're using the Intel graphics, right, and I assume you have an Intel graphics driver installed?

Have you used the Intel HD Graphics Control Panel software, that is installed with the Intel graphics driver?

If not, run it and set the Refresh Rate in the General Settings, selecting the appropriated monitor when you do.

No idea what CPU and version of Windows you use, but this is a listing of the latest Intel graphics drivers:

I read in a review that only the DisplayPort input supports 60Hz on this monitor, and HDMI is limited to 30Hz. LG's specs are ambiguous in this area, which normally means not good news.

A simple cmos reset seems to have brought the HDMI 2 port back to life. Still, I am stuck at 30hz.

I bought the board specifically to drive two 4k displays at 60hz (one on display port, one on HDMI2). This never worked on HDMI2 (no problem with display port). I am using stock HDMI cable supplied with the 27UD68-w monitor. 30hz has been the best I could do.

I flashed to latest BIOS, no change. loaded latest HDMI firmware (the one it defaults to in case there are more), no change. Now, suddenly, HDMI2 stopped working altogether, and when I try to load the firmware again, the loader says "ISP failed" or something to that extent.
 
Please help.
